Service Coordinator opening with New Vision™ Service

New Vision™ is an inpatient medical stabilization service for adults with drug, alcohol, and related health issues. The service accepts appropriate adults, who are currently using or are experiencing acute withdrawal symptoms from certain drugs. New Vision™ has a fully qualified treatment team including an attending physician and nursing personnel who utilize a multidisciplinary team approach in the stabilization of the patient.

Position Purpose

This position is responsible for performing a variety of social service duties as explained in the Job Description and as may be assigned by supervisor. The Service Coordinator is responsible for the management of the New Vision™ office, which includes participation and oversite of the admission and referral process as well as internal documentation auditing. In addition the Service Coordinator is responsible for planning weekly outreach in the community to enhance the visibility of the New Vision Service and client hospital. The Service Coordinator works collaboratively with hospital administration, physician and nursing staff, Regional Operations Manager, and Clinical Director to coordinate a continuum of care for the addicted patient. Employee is expected to be present at client hospital during assigned business hours to supervise the intake staff, assist with inquiries, admissions, discharge planning, meet with hospital physicians, nursing staff, and administration as well as provide general direction on the proper functioning of the site. Service Coordinator is expected to be on site daily unless they are off site performing community outreach and educational activities pursuant to the marketing and outreach plan.

Qualifications

Education:

Minimum of a Bachelor’s degree in related fields such as Human Services, Psychology, Counseling, Substance Abuse, Criminal Justice, and Social Work.
Licensed/Certified professionals preferred.

Required Knowledge:

  • Basic Understanding of chemical dependency
  • Understanding of HIPAA guidelines and policies
  • Proficient in Microsoft Office (Word, Excel, PowerPoint, etc.)
  • Understanding of email and attachments
  • Understanding of documentation as it pertains to the healthcare industry

Preferred Experience:

  • Minimum of at least three (3) years of social service and/or clinical service work
  • Marketing or Public Relations experience
  • Ability to work with a diverse population of people
  • Ability to demonstrate strong communication and organizational skills
  • Ability to demonstrate effective leadership and management skills

Skills/Abilities:

  • Must be proficient in Microsoft Word, Excel, Power Point, and able to navigate email.
  • Ability to maintain confidentiality of patient information, policies, procedures, protocols, and other such sensitive information.
  • Ability to remain open to other’s ideas and exhibit a willingness to try new things.
  • Ability to speak clearly and persuasively in both positive and negative situations.
  • Strong reading comprehension and grammatical knowledge
  • Ability to present numerical data effectively
  • Strong time management skills
  • Ability to identify problems and develop realistic action plans

Essential Functions of the Position:

  1.   Screening and Intake
    Effectively screen, verify, and report patient information

  2.   Management of Employees
    •  Oversee the Intake Coordinator(s), using principles of management and supervision of staff, problem solving, conflict management, and team building skills
    •  Ensure that inquiries are effectively converted to admissions; audit patient charts; ensure timekeeping policy is followed
    •  Ensure that Compliance Program is maintained at site
    •  Seek experiences and continuing education/training that will enable both Service Coordinator and Intake Coordinator to work more effectively in the field of chemical dependency
    •  Act in a manner that demonstrates leadership and will ensure that code of conduct is continually upheld by both Service Coordinator and Intake Coordinator

  3.   Function as a Department Head
    Functions as a liaison between the client hospital and SpecialCare Hospital Management Corporation in order to coordinate the operation of the New Vision™ Service effectively.

  4.   Community Outreach & Education
    Complete Community Outreach expectations by acting as liaison between community and New Vision™ Service

  5.   Site Performance:
    Ensures that staff and site meet performance standards. Ensure that documentation standards are upheld in all areas. Ensure that all activities are conducted in accordance with the company’s compliance program and policy and procedure guidelines.

  6.   Communication Skills:
    Utilizing necessary verbal and non-verbal skills necessary to support the success of an efficient and effective site. Demonstrate effective verbal and listening skills when interacting with supervisors, peers, hospital administration, hospital staff, patients, patient families, and community at large.
