Intake Coordinator – Detox Program Admissions
Responsibilities:
  • Screen & assess patients via phone and in person; initiate insurance verification; scheduling assessment and intake
  • Complete all paperwork clearly, concisely, and in line with SpecialCare policy and procedure, as well as applicable state & federal law such as HIPAA
  • Effective communication with hospital physician & nursing staff, regarding patient assessment, continued stay, and discharge planning
  • In facility round and follow up calls on patients daily according to policy and procedure
  • Developing tailored discharge plans for patients who have completed treatment in order to coordinate a continuum of care for the addicted patient
  • Collaborating and reporting to SpecialCare Operations & Clinical teams as required
Requirements:
  • Bachelor’s degree in Human Services, Psychology, Counseling, Substance Abuse, Criminal Justice, or related field with at least two (2) years of social service and/or clinical service work
  • Must have knowledge of addiction and/or dependence (substance abuse counselor and/or case manager; previous experience in substance abuse withdrawal, MAT Treatment, or detox a plus)
